Once your out-of-pocket Medicare expenses reach a threshold in a calendar year, Medicare pays a higher proportion (80–100%) of remaining out-of-hospital costs for the rest of the year.

What you get
Extended Medicare Safety Net covers 80% of out-of-pocket costs once you exceed the threshold (~$2,184 for families/concessional, ~$770.30 for others). Original Safety Net covers 100% of remaining Medicare Schedule fee gap.
